Where to Begin

To access our personal care services, it’s important to have full Medicaid affiliated with insurance providers such as Blue Cross Blue Shield, Molina Healthcare, Presbyterian Healthcare, or United Healthcare. Simply reach out to your insurance provider to schedule an in-home assessment, which determines your eligibility and the weekly hours approved for assistance. Once qualified, we hope you’ll entrust Ideal Home Care with your personal care needs. Rest assured, we’ll guide you through the process seamlessly, providing compassionate support every step of the way.

Those without Medicaid can call the New Mexico Aging and Long-Term Services to schedule a Medicaid waiver appointment @ (800) 432-2080.

Our Services

Personal care services encompass a range of services aimed at assisting individuals with daily living activities, especially those who may have difficulty performing these tasks on their own due to age, illness, disability, or other conditions. These services are often provided in the home or in community settings and can be customized to meet the specific needs of each individual. Common personal care services include:

Assistance with Activities of Daily Living (ADL’s):

  • Bathing

  • Dressing

  • Grooming

  • Toileting

  • Eating

  • Mobility and transferring (e.g., from bed to wheelchair)

Help with Instrumental Activities of Daily Living (IADLs):

  • Meal preparation and planning

  • Light housekeeping and laundry

  • Shopping for groceries and other essentials

  • Transportation

Companionship and Emotional Support:

  • Engaging in conversation and recreational activities

  • Providing emotional support and encouragement

  • Accompanying clients on outings

Respite Care:

  • Temporary relief for primary caregivers, allowing them time to rest or attend to other responsibilities.

Specialized Care:

  • Care for individuals with specific conditions such as dementia, alzheimer’s disease, chronic illness, post-surgical care, and assistance during recovery periods. These services can be essential for maintaining the independence, dignity, and quality of life of individuals who require assistance.

The Consumer-Directed Model is an approach to personal care services where the individuals receiving care (often referred to as members or consumers) have greater control and decision-making authority over the services they receive. This model empowers members to direct their own care according to their preferences and needs. Here are the key aspects of the Consumer-Directed Model:

Choice and Control:

  • Consumers have the independence to choose their caregivers, which may include hiring family members or friends. If necessary, members can also terminate caregivers.

  • Consumers decide how, when, and where services are provided, tailoring their care to their specific routines and preferences.

Customized Care Plans:

  • Members can modify their care plans as their needs and circumstances change.

Flexibility and Responsiveness:

  • The model emphasizes flexibility, allowing members to adjust their care arrangements quickly in response to changes in their condition or preferences.

The Consumer-Delegated Model is a variation of consumer-directed care that combines elements of both traditional agency-directed and consumer-directed approaches. In this model, consumers (also referred to as members or clients) have a significant role in directing their care but delegate some responsibilities to Ideal Home Care. Here are the key aspects of the Consumer-Delegated Model:

Shared Responsibilities:

  • The agency selects the caregiver with the Consumer’s approval.

Personalized Care Plans:

  • Care plans are developed collaboratively, with input from the consumer.

  • Plans are tailored to the specific needs, preferences, and goals of the consumer.

Support and Training:

  • Consumers receive support and training to effectively manage their care and make informed decisions.

  • Ideal Home Care provides additional training and resources to caregivers to ensure they deliver quality care.

Quality and Compliance:

  • Ideal Home Care ensures that the care services meet quality standards and comply with relevant regulations and policies.
